IC Program Services and Outreach Materials

Injectable Contraceptive (IC) service and outreach materials are available for both the pharmacy and the clinic or physician’s office. IC services are marketed to consumers under the name “Health Step” and women can also obtain information about this service, including pharmacy and provider locations, through a toll free hotline 800-323-1336.

Injection Card

Contains patient information (name, DOB, provider, method, and coverage) and a place to record injections.

The Injection Card is initiated at the clinic or physician’s office, and taken by the patient to the pharmacy.

The pharmacist needs both the prescription and Injection Card to provide the IC service.

IC Encounter Form

Used by the pharmacists when providing IC services.

Patient completes the top portion; bottom portion functions as service record.

Health Step Brochure

Explains to consumers how to participate in the IC program.

May be used at the clinic or physician’s office and the pharmacy.

Depo-Provera Info Sheet

Provides an overview of the use of Depo-Provera.

Pharmacists provide this brochure to patients when administering Depo-Provera injections.

Health Step Poster

Displayed in participating pharmacies and health clinics to advertise the Health Step program to consumers.

 

Referral Resources

A list of references pharmacists may provide to patients for additional community resources.
